Volunteer Waiver

In order to assure the safest site for volunteers and to protect the physical health of the volunteers, we ask that every volunteer attend and orientation, if possible, watch our safety video and sign this waiver and return it to the administrative offiice at 1860 NE 4th Street, Bend OR 97701.
